Tips for using it
Get the problem statement right first. A vague or broad starting point leads to vague answers. Keep it to one sentence, grounded in something specific and observable.
Stay with the discomfort. The first two "whys" usually feel easy. The value comes from the later questions where you have to think harder. Resist the urge to jump to solutions too early.
Focus on causes, not blame. If your chain of whys leads to "person X didn't do their job", step back and ask what made that outcome likely. Systems and conditions are almost always more useful targets than individuals.
