Be strict with the two questions - important means it moves your goals forward, urgent means it genuinely can't wait. Not everything that feels pressing is either, and if everything lands in Do, the sort isn't finished: your definitions have slipped.

Redo it weekly rather than once - the quadrants don't hold still, and this week's Decide becomes next month's crisis if it's left long enough. And when you run it with a team, spend the time on the disagreements: two people putting the same task in different quadrants is the conversation that's actually about priorities.
